Flyback
NEC - should work for the TM-623 or TM-600 B&W monitors - has 8 terminals - the original flyback was part number TFB-1006AS.
Note that pins 1 and 5 appear to be shorted - and 5 goes to common.
The photo shows a pull from a working chassis, we have two NOS flybacks and two pulls, pulls are discounted only 10% and were from working monitors.
